Hello: Conversations for Living and Dying Well is a game to start us thinking, talking, remembering, imagining, laughing and yes, sometimes tearing up, as we consider questions about what might be important to us at the end of our lives. &nbsp;It is a game that gets us talking about what makes life meaningful and starts advance care planning conversations.</p>=0D=0A=
<p><strong> What is Advance Care Planning?</strong> Advance Care Planning is the process of thinking, talking about, and recording the health care choices we would want made for us if we couldn&rsquo;t speak for ourselves.</p>=0D=0A=
<p><strong>Why do Advance Care Planning???</strong> Think of advance care planning as a gift we leave for the people we love, so if they're ever in the position of having to make health care decisions for us, they know they are in line with what we would want. That can make a difficult situation a little bit easier and leave them feeling they are honouring who we are. It can be the difference between caregivers doing things FOR us instead of TO us. Even if we can speak for ourselves towards the end of our lives, why not put a little thought now into what we might want, so if we find ourselves in a tough spot we have already done some of the thinking. And remember, we can ALWAYS change our minds, so no worries about putting our thoughts down on paper now.</p>=0D=0A=
<p>An Advance Care Plan helps prepare us and our families for what we may be choosing later on. As Kathy Hagerty, a Cowichan Hospice volunteer, so aptly put it: &ldquo;The main lesson I learned was to start expressing how I feel NOW instead of when I am almost at my expiration date and probably not able to express much of anything.&rdquo; Who knew that something as simple as playing a game could be a gift to your family, help you live a better life... and BE FUN?????</p>=0D=0A=
